Dr. Gawande’s Respect for Patient Autonomy in Whose Body...

In his article â€Å"Whose Body Is It, Anyway?†Ã¢â‚¬â€appearing in the New Yorker in October 1994—Dr. Atul Gawande highlights the complexities of the doctor-patient relationship vis-à  -vis patient autonomy and decision-making. Dr. Gawande explains that a respect for patient autonomy (i.e., allowing patients to choose between courses of treatment, therefore directly influencing their health outcome) is the â€Å"new normal† for medical practitioners. However, Gawande also contends that there are times when patients are better served by a voluntary relinquishment of that autonomy. Dr. Gawande shares a personal story with the reader to make his case. He tells us about his daughter Hunter, who suddenly stopped breathing at 11-days-old. Dr. Gawande’s wife quickly noticed the respiratory stoppage, shook Hunter, and her breathing started anew. The Gawande family immediately took Hunter to the hospital where, in an environment guided by the â€Å"orthodoxy of patient autonomy†, a team of physicians presented them with different treatment options and asked them to choose their preferred course of action. Dr. Gawande was a trained, licensed, practicing physician – surely he would be better equipped than most to make such a decision. EFFECTS OF WATCHING TOO MUCH TV Discoveries and invention of devices are always welcome till we, humans, find a way to abuse its benefits and be adversely affected by it. Mexico needs a new political economy designed by Mexican economists and politicians, people who are committed to Mexico and to the future of its society. CHAIN ORGANIZATION Another type of cause-and-effect analysis is the casual chain. Unlike the multiple cause-effect analyses, the causes and effects in a casual chain are always directly related; in fact, they are linked. In the casual chain, one effect can become a cause of another effect, which in turn becomes a cause of another effect, and so on. For example, let us say that a man had problems at work; as a result, he started to drink heavily. His heavy drinking eventually caused him to behave in an abusive way to his wife; his wife then alienated herself from him. His wife’s alienation caused him to feel even more alone and more of a failure; his depression caused him to decide for a divorce. UPSETTING THE BALANCE OF NATURE The members of a living community exist together in a particular, balanced relationship, or ecosystem. One animal species eats another animal species which in turn eats another. Over years, a balance is worked out among the plants and animals in a community and it remains basically stable. It is like a huge puzzle with all of the pieces in their proper places. However, at times this balance in nature is disturbed, resulting in a number of possibly unforeseen effects. Perhaps a disease results in the near extinction of one species, leaving another species with no natural predator. The result can be a terrific increase in that one species’ population. This could further result in the devastation of a shared food supply, which could in turn affect another species. It is possible for the disruption in the balance of nature to have natural causes: disease, drought, fire. Sometimes, however, human beings intervene in a natural environment, perhaps only slightly and with good intentions. The result is the same. The balance of nature becomes unbalanced and results in an entire chain reaction of unforeseen and unwanted effects. A good example of this occurred in the Antilles in the 1870’s. Sugar cane was a major crop there, but rats were eating and nesting in the cane, causing a great deal of damage. The mongoose, a one-and-a-half-foot-long mammal of the East Indies, was known to be an excellent rat hunter. Several males and females were imported in 1872, and laws were established that forbade the killing of them or their offspring. The mongoose flourished in the Antilles. After ten years it had multiplied abundantly and had significantly reduced the rat population. Consequently, damage to the cane fields was greatly reduced. It seemed that the scheme to add another piece to the ecological puzzle in the Antilles had been successful. However, that is not the end of the story. The influence of the mongoose did not stop there. As the rat population decreased and the mongoose population increased, the mongoose needed to enlarge its menu. It attacked young pigs and goats, game, poultry, and began to destroy bananas, maize and pineapples. Because the mongoose could not be hunted, its numbers increased rapidly, and it became a terrible past. All of the indigenous animals suffered damage. The mongoose learned to enjoy the native birds, snakes, lizards, turtles and their eggs. Now, it was specifically these animals that kept the local insect population in check. There were in the ecosystem of the Antilles a number of beetles, borers, and other insects that lived on and in the sugar cane. Until that time, they had not caused significant damage to the cane, because they were the natural food of so many local animals that kept their numbers down. However, as the birds, snakes, lizards, and turtles disappeared, the insect population began to increase. With no natural predators to keep them in check, the insects began to do more and more damage to the cane fields. Finally, the people of the Antilles realized that the introduction of the mongoose had caused a finely and delicately balanced system to go awry. The law against killing the mongoose was rescinded, and the mongoose population was reduced. Gradually, the different members of the plant and animal community came back into balance with each other and equilibrium was reestablished. However, the human members of the community would not soon forget that a single change in an ecosystem can cause a chain reaction that results in completely unforeseen and sometimes unwanted effects. PRACTICE: MAKING AN OUTLINE The following short essay describes a simple CHAIN reaction. SAD When winter arrives, some people get sad, even depressed. Doctors have recently started to study the causes of a medical disorder that they have appropriately named SAD, or seasonal affective disorder. Since absence of light seems to be the cause of this disorder, a daily dose of light appears to be the cure. Doctors advise patients to sit in front of a special light box that stimulates natural light for a few hours every day. We each belong to an entire collection of cultures different shared values, beliefs, and attitudes. Our own actions influence the way we characterize the actions of others. It is not easy to understand people from different cultures. In forming an opinion of other cultures, we have a tendency to make assumptions and generalizations which can get in the way of common sense. More often than not the technical language is understood, but other elements of communication are overlooked. Most of all communication is nonverbal. As much as 90% of your communication is done without words. Gestures, facial expressions, and posture provide information about a persons emotions and relationships with others (University of Northern Iowa, n.d.). Nonverbal communication is used more than verbal communication. The majority of people identify others by first appearances without communicating. It is easy to misunderstand another cultures expression of respect or affection. When you encounter someone new, the expectation of the other person perhaps is a nod of the head, a handshake, or a hug. How is a person to know? To have a successful business environment it is essential to have excellent communication. â€Å"There is no better way to build trust than through communication† (Hayhoe Grady, 2008, p.40). Eight years ago while working for a financial institution, I had a miscommunication with a coworker. Keiko was who is originally from Tokyo and worked as a finance officer for the local branch. Although Keiko has been in the United States for several years she still had a very heavy Asian accent and was incapable of forming proper grammar, but she was an excellent worker and expert at loans. If someone could not comprehend what she said they would then ask her to say it again and she would without a problem. Well a client came in one day and asked for her because he had a question a loan for their business. I went ahead and called her over for the client to ask her a question in reference to his business loan. The client did not comprehend what she was saying and Keiko did not unders tand what the needs of the client were. After explaining for the third time what he wanted the client became aggravated. A bit frustrated the client exasperatingly stated, â€Å"I cannot comprehend why businesses hire people that who do not speak English. Don’t they know that they work and live in America and need to speak English! If we lived in their country we would have to learn their language.† To make matters worse, I expressed my regret to the client and told him that I would talk with the loan manager to make sure that his work with our manager and make sure that his demands were taken care. After the client left the branch, Keiko began to cry and blamed me for what had occurred. I became a little offended and did not comprehend what was happening, I was taken aback. My thoughts were what did I do and why was she upset with me? I thought I was doing her a favor by getting the client away from her. This one example of why it is important to have Intercultu ral Communication in the workplace. I did not think there was anything wrong with what I did think what I did was wrong; on the other hand, evidently that is not what Keiko believed. When I talked with the loan manager and explained to her what took place, I realized that Keiko was sadden because I did not support her when the client made those malicious comments about her and she also thought I should have explained the issues the client had to her myself. I learned from her that day some Asian cultures. She explained to me that in China, Japan, and other Asian cultures it is imperative to avoid causing your counterpart to lose face. In Asian cultures to raise your voice or shout at a person in public, or to correct them in front of their peers will cause them to lose face (Berman, 2003). I made an apology to Keiko and let her know I was trying to be helpful and that I meant no disrespect. One of the crucial points to effective intercultural communication is knowledge. It is extremely vital that people recognize the problems of intercultural communication and make an effort to overcome these problems when there is a culturally diverse workforce. A large number of people make the assumption that cultural diversity is the cause of communication issues. Employers should be willing to be open-minded and forgiving, rather than threatening and aggressive, if issues arise. One should be cautious in intercultural interactions and come to any assumption of how a person feels or what person thinking. One of the main points to intercultural communication is Reflective Listening. Reflective Listening is used to analysis what someone is saying by reiterate what you think you heard. This helps in validating what was heard is accurately understood (Vuckovic, 2008). Reflective Listening is also helpful because there are many words and gestures that are used in different ways among languages or cultural groups. Some organizations frequently use mediators that are familiar with different cultures. This can be very helpful in intercultural communication situations. Mediators help in translating both the words and gestures. For example, what would be considered inappropriate in one culture can be discussed and changed, so it does not offend before they are shared with culture. Americans are quick to reference or direct to the issues. Other cultures like to set up relationships before any business is discussed. If a relationship is not made before discussions on business are started there is no trust in the person that is speaking. A mediator that recognizes the issue can help in making appropriate adjustments to the procedures. Organization need to be knowledgeable about managing a diverse culture. It is vital for an organization to succeed in global arena. The responsibility of a diverse culture influences talent management policies and procedures in the workplace. One talent continues to motivate others; demonstrate the ability to effectually communicate with clients, coworkers, and management.
